چکیده

The rapid growth in the number of the elderly has created an unprecedented challenge to the current health system. Taking care of the frail elderly continue to face significant barriers achieving the desired and optimal care. The elderly are most vulnerable for higher rate of complications in the post acute care of their illness either due to mental, physical or psychosocial impairments, requiring multiple interventions needed on many fronts. Skilled nursing facilities are an essential part of the post acute geriatric care. Post acute care (PAC) encompasses a wide range of health care services that share the goal of restoring recently hospitalized patients to the highest level of functioning possible. Patients can access PAC services in a wide range of settings, including skilled nursing facilities (SNFs), inpatient rehabilitation facilities, long-term care hospitals, and in their own homes, with services from home health agencies [1] In this brief review, we will focus on key elements in the skilled aspect of the post acute care as a merging factor in the long term nursing facilities. *Corresponding author: Yahya Murad, Geriatrics Center Clinics, University of Michigan Health System, USA, Tel: 734-936-2519; Fax: 734-647-8535; E-mail: [email protected] Received September 19, 2011; Accepted October 27, 2011; Published October 27, 2011 Citation: Murad Y (2012) Skilled Nursing Facilities and Post Acute Care. Nurses' Role in Managing "The Fit" of Older Adults in Skilled Nursing Facilities.

Post-acute care for older adults often involves transfer to a skilled nursing facility (SNF) following hospital discharge. There is a dearth of evidence regarding the role of nursing and its potential to ...

Editor's Note The Role of Post–Acute Care in Variation in theMedicare Program Variation is frequently cited as evidence of unnecessary or wasteful health care, because we would expect a patient’s medical condition, not geography or health care professional, to dictate who receives a particular procedure or service. Understanding howmuch variation is owing to regions vs providers within regions ...

The dilemma of vacant beds: Did liberal CON policies toward hospitals make nursing homes ’sick’?

Hospital regulators use certificate-of-need (CON) licensing for multiple purposes and objectives. In an attempt to relieve financial distress of acute care hospitals in Florida, CON restrictions on skilled nursing home care were relaxed during much of the 1990s to allow hospitals to provide lucrative post-discharge care under Medicare. The consequences of this policy for freestanding skilled nu...
